This form can only be submitted once. You may keep the browser window open as long as you need (e.g., filling it out intermittently over the course of a day). However, after you submit your data you will not be able to go back to modify answers or add information. Before you begin, please note that you will be asked the following types of retrospective information:

• Year of ABOS REcertification
• Fellowship topics (if you ever had a fellowship)
• Average hours worked per week
• Vacation time
• Days away from practice on orthopaedic business or attending education/training
• Distribution of your professional time (clinical care/teaching/research/administration/other)
• Percent of sources of payment (Private/HMO/Medicare/Medicaid/Workers Comp/Charity)
• Income (gross, net; % from Workers Comp/IMEs)
• Typical number of surgeries/month
• Typical number of specific procedures performed per month for: carpal tunnel release; knee arthroscopies; spine re/fusions; ACL repairs; total/partial/revision hip arthroplasties; uni/bi-tri/revision knee arthroplasties; rotator cuff repairs; spinal disc replacements

Please do not feel compelled to do an exhaustive review to respond to these items; while we strive to obtain information that is as accurate as possible, we understand that your time is valuable. If it is impractical to readily supply these numbers, please make a "Good Faith Estimate" of your experiences as a practicing orthopaedic surgeon.
