Failure of a reverse total shoulder arthroplasty presents an extremely difficult problem for even the most experienced surgeon. This video introduces a novel technique for salvage of a failed reverse total shoulder arthroplasty. A clinical case demonstrates revision of the failed reverse shoulder prosthesis to a hemiarthroplasty using the yoke technique in a patient with a failed glenosphere baseplate and inadequate glenoid bone stock to support a prosthetic component. This technique uses an Achilles tendon allograft to resurface the remaining glenoid bone and stabilize the proximal humerus. The allograft provides a smooth surface for the hemiarthroplasty to articulate, and it recreates the capsuloligamentous structures of the shoulder, which prevents anterosuperior escape.
