Practice Management reSOURCE Directory Endorsement Form
Information you provide is confidential; it will not be shared with any third parties.
Because the AAOS wants to ensure that the Practice Management reSOURCE Directory is populated with reliable companies, every listed company must obtain endorsements from three (3) satisfied orthopaedic practice clients. The endorsements must be completed by orthopaedic practice clients whose group includes AAOS members.
There are two (2) options for obtaining the necessary endorsements:
| Option One: | Complete the first section of the form and email or fax the form to the three (3) orthopaedic practice clients and request that they complete and sign the endorsement form. Prior to sending the endorsement form, we recommend you contact each practice and secure their approval for the endorsement. |
| Option Two: | Please complete this form and email or fax it to AAOS. Provide your company information and the names and contact information for three (3) different orthopaedic practice clients. The AAOS will contact these individuals to secure the necessary endorsements. Prior to submitting this information, we recommend you contact each practice and secure their approval for the endorsement. |
