Leadership Fellows Program
Welcome to the Leadership Fellows Program (LFP) Application and thank you for your interest in participating in the LFP. I hope that you will follow through and complete the application to become a Leadership Fellow or Mentor. To be eligible as an LFP Fellow you must be an AAOS Fellow and age 45 and under as of January 31, 2009.
The application that follows has several sections that require you to complete:
- Personal Statement – Note: this is very important as the LFP selection committee gives significant weight to your statement.
- References – You will need letters of reference from two AAOS fellows. Note: You will need to contact your references and ask them for the reference letter. A guide form for drafting the letter of reference is downloadable from this site.
- The remaining sections ask you to provide professional and personal background information.
The LFP is an exciting and interesting program. I strongly encourage you to complete an application.
