The AAOS creates evidence-based clinical practice guidelines (CPGs) to improve patient care by outlining the appropriate information-gathering and decision-making processes involved in managing various orthopaedic conditions. CPGs are an educational tool that may guide qualified orthopaedic surgeons and other physicians through a series of diagnostic and treatment decisions in an effort to improve the quality and efficiency of care. AAOS guidelines should not be construed as including all proper methods of care or excluding methods of care reasonably directed to obtaining a satisfactory patient outcome. The ultimate judgment regarding any specific procedure or treatment must be made in light of all circumstances presented by the patient and the needs and resources particular to the locality or institution. In June 2009, the AAOS released the guideline on the treatment of Pediatric Diaphyseal Femur Fractures. The work group systematically reviewed the available literature, evaluated the level of evidence found in the literature, and subsequently wrote the recommendations based on a standardized process.

This guideline addresses the treatment of isolated diaphyseal femur fractures in children who have not yet reached skeletal maturity. The guideline provides information on pediatric patient management after diagnosis of a diaphyseal femur fracture. The guideline is not intended for use in pediatric patients who present with additional coexisting injuries that require formal surgical intervention or other life-threatening conditions that take precedence over the treatment of the diaphyseal femur fracture.

The annual rate of children who present with femoral shaft fractures has been estimated at 19 per 100,000. Boys have a higher risk of fracture than girls and this is consistent with participation of boys in sporting activities.

Diaphyseal femur fractures account for 1.4% to 1.7% of all pediatric fractures.

There are many components to consider when calculating the overall cost of treatment for pediatric femoral fractures. The main considerations for patients and third party payers are the relative cost and effectiveness of each treatment option. But hidden costs for pediatric patients must also be considered. These costs include the

  • additional home care required for a patient,
  • the costs of rehabilitation and of missed school for the patient,
  • child care costs if both parents work, and
  • time off of work required by one or both parents to care for the pediatric patient.

The AAOS Guideline addresses the treatment of isolated diaphyseal femur fractures in children who have not yet reached skeletal maturity. The guideline provides information on pediatric patient management after diagnosis of a diaphyseal femur fracture. The PDFF guideline is not intended

  • for use in pediatric patients who present with additional coexisting injuries requiring formal surgical intervention or
  • for other life threatening conditions that take precedence over the treatment of the diaphyseal femur fracture.

An important segment of this webinar is an explanation of how the Work Group evaluated the medical literature to formulate the guideline recommendations.
